    How do I request a transcript?

    There are four options when it comes to ordering transcripts:

    • You may come in person to Burke Administration Building, Room 102
    • Mail your request to ONU, Registrar's Office, One University Avenue, Bourbonnais, IL 60914  
    • Fax your request to 815-935-4992 (must include credit card information and your signature) 
    • Scan the completed form and email as an attachment to

    Why can't this be done electronically?

    The key to a transcript request is your signature. It is this one point that prevents us from accepting either email or telephone transcript requests.

    What information should I include?

    Along with your signature, please include the following:

    • Your complete name now and all name changes
    • Your social security number
    • Your dates of attendance
    • Address where you want your transcript sent
    • Some form of payment. Normal processing fee per official transcript is $5.  24-hour (one business day) processing per request is an additional $5. We accept Visa, MasterCard, and Discover credit cards. Include your account number, the name on the card, the expiration date, and your daytime phone number with your fax request.

    How long will it take?

    Transcripts are mailed within 3-5 working days following receipt, unless 24-hour processing is requested and paid for. We do not fax transcripts.

    What is an "official" transcript?

    Official transcripts are printed on SCRIP-SAFE security paper with the name of the University printed in white type across the face of the document. A black-and-white document is not official. Official transcripts are sent directly to an employer, another institution, or to another third party on behalf of the student. If the transcript is released to the student, it will bear the stamp "Issued to Student" on the face of the document. This copy is still an "official" document.
